Employee Disciplinary Action Documentation Form – HR Template
Employee Disciplinary Action Documentation Form
Introduction
An Employee Disciplinary Action Documentation Form helps HR teams maintain a structured record of workplace incidents, policy concerns, fact-finding, employee responses, review outcomes and any corrective action taken.
Unlike a warning letter, which formally communicates a concern to an employee, this form is primarily an internal record used to document the handling of a case.
This template should be adapted to the organisation's disciplinary policy and applicable employment laws. It does not replace any required investigation, hearing or employee response process.
